SystmOnline is a great online service which allows you to book appointments, order repeat prescriptions and view read coded information held in your medical record.
Register at the surgery in person
Step 1. read this information leaflet (PDF, 336KB)
Step 2. download, complete and print our application form (DOCX, 537KB)
Step 3 Visit the Practice in person and hand to reception your completed application form. Please inform the receptionist whether you've read the information leaflet.
Please Note that we also have the application form and information leaflet at reception if you are unable to download, complete and print it.
Step 4 Show your Identification - two forms of ID. one photo ID such as Passport, drivers licence, freedom pass etc and also one form of ID with your home address such as recent utility bill, bank statement etc. Full list of acceptable ID (PDF, 47KB)
Practice Support Workers/Admin Team will be able to grant access to patients who present with the correct identification, for appointments and medication same day.
If patients request access to their detailed coded information they will be notified that it may take the practice up to 21 days to review their application and grant access if appropriate. This is a guide only and in some circumstances may take longer.

Important Information: Proxy Access for Children
Patients aged 16 and over must apply for this registration form in person. Applications can not be made for anyone other than yourself unless the patient is under 11 years of age and you are the patient's parent or legal guardian. In this case an application form will be issued but you will need to remember to log in using the child's SystmOne online details if you are making an appointment or requesting a prescription on their behalf.
On the child’s 12th birthday, GP computer systems will automatically restrict the scope of existing proxy access. Parental proxy access may be reinstated if, after discussion with the parent(s) requesting access, the child’s GP believes that proxy access would be in the child’s best interest.

If you already use online access you can continue to use it. You can use the NHS App as well.
